To file a Citizen's Complaint you will need to fill out the packet below and submit to the court. At a minimum the court will need the packet filled out completely (all eligible fields) and signed by complainant. Applicable evidence can be attached and submitted to the court along with completed packet, but is not required.
Examples of possible attachments:
- Pictures of damage/injuries/subject in question
- Video of alleged actions (on USB or disk)
- Witness statements, make sure that the witness information in packet is completed
- Receipts or estimates for damage and/or repairs
After your packet is received it will be reviewed by the city prosecutor, along with any other evidence that is submitted with it, for further recommendation to the court.
